How to Refer Patients

To refer a patient to Pasadena CyberKnife Center, please call us at (626) 768-1021. You can also print out and complete our Patient Referral Form, then fax it to us at (626) 768-1022. Once we receive your form, we will call your patient and schedule an appointment. Thank you in advance for your patient referral!
